Patient Corner

  • How do I make an appointment?

    To make an appointment, give us a call at 312-943-7850. You can also request an appointment on our website or at ZocDoc.com.

  • Do I need a referral?

    No, you can make an appointment with or without a referral from a primary care physician.

  • What is your cancellation policy?

    We ask all patients to cancel 24 hours in advance. The practice reserves the right to invoke a non-refundable fee for failure to cancel prior to that time period. If you fail to appear for your appointment we will contact you to follow-up and make sure you are okay. You are always welcome to reschedule.

  • How do I inquire about payments?

  • Do you file my insurance claim?


  • What if I don’t have insurance?

    If you do not have insurance we will be glad to bill you fee for service and work out a payment schedule with you if necessary

  • How should I prepare for my visit?

    Be aware that you will probably have x-rays, so leaving jewelry and valuables at home is best as they will have to be removed. You may want to bring a pair of shorts so you are most comfortable during the x-ray process. If you already had x-rays or an MRI taken somewhere else, make sure to bring your disc with you.

    If you are coming for physical therapy feel free to bring workout clothes.

  • How can I get a copy of my medical records?

    You can request them at the front desk. Please be aware, some records require a fee.

  • What if I need to get in touch with my physician and the office is closed?

    Call the office at 312-943-7850 and the answering service will contact the physician on call. However, always dial 911 if you are having an emergency.

Northwestern Center for Orthopedics

Northwestern Center for Orthopedics Contactus allows you to send Non Urgent, secure messages to doctors with whom you have had an office visit within the last three (3) years. Generally, messages are first received and reviewed by a nurse or other member of your care team prior to a physician. 


Please be advised messages sent via or to Contactus are only viewed during business hours. Northwestern Center for Orthopedics will make its best efforts to respond to messages within two (2) business days. Messages sent via Northwestern Center for Orthopedics Contactus will become part of your permanent medical record. Current and future Northwestern Center for Orthopedics staff involved in your care will be able to view your messages.

The security of your health information is extremely important to Northwestern Center for Orthopedics. We go to great lengths to make sure your private health information remains safe and secure. We protect your medical records as required by state and federal laws governing the security and confidentiality of medical records. We use technology to encrypt your personal information such as User Name, Passwords, and account information over the internet. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased.
